Specialist Trauma Informed Resettlement Services (NHS Hub)
The PDS is a service across Yorkshire and The Humber that offers case review and signposting to local clinical commissioning groups and specialist commissioners for clients that are detained in (or are at risk of escalating into) secure hospital beds, or where pathways back to the community are blocked leaving clients ‘stuck’.

About the service
The overarching aim of the Pathway Development Service (PDS) is to support systems striving to deliver trauma-informed, integrated care for people who experience the world as profoundly unsafe (often associated with the diagnostic label of a ‘personality disorder’).
The team works in collaboration with the wider PDS multi-disciplinary team, primarily with clients with high risks and complex needs, many of whom have attracted diagnoses of personality disorder.
Housing and Resettlement Caseworkers offer housing reviews to any clients where housing routes to the community are challenging. They negotiate and consult with suitable supported housing placements and work closely with providers to upskill their teams to support clients in a trauma-informed manner.

Emerge are a Leeds city-wide multi-disciplinary service that offers care-coordination, psychology and group-work programmes to people that have experienced complex trauma and adverse life events that have led to emotional and interpersonal difficulties. Many of the clients have attracted diagnoses of personality disorder, however they will work with anyone who has developed ways of coping and surviving that result in harm and distress, recognising that many clients find the label of personality disorder stigmatising and distressing.
Inspire North has a housing and resettlement caseworker resource embedded within Emerge, that supports the multi-disciplinary team to work with clients where their housing is impacting on their wellbeing and recovery. They work closely with the Accommodation Gateway for clients admitted to the acute wards in Leeds, as well supporting clients in the community, to assess housing need and broker placements that will meet their needs.
The caseworker will work with a client prior and post settlement to reduce tenancy breakdown and increase the success of community integration. The caseworker can offer bespoke training to housing providers to support the understanding of a client’s formulation and how to work in a trauma-informed manner to ensure the success of the placement for the client.

Discovery is a partnership between LYPFT and probation services in Yorkshire and the Humber and is part of the nationwide Intensive Intervention and Risk Management Service (IIRMS). Community Links has housing and resettlement caseworkers embedded in LYPFT, who offer specialised housing support to high risk clients with complex needs and offending histories.
The caseworkers work within a psychologically-informed framework to support male and female offenders, many of whom have experienced significant trauma and have attracted diagnoses of personality disorder as a result, to broker suitable housing placements and to resettle clients. Many of the clients have served significant prison sentences or are detained under Indeterminate Public Protection (IPP) sentences, so pathways for these clients into the community can be challenging.
The caseworker supports housing providers to appreciate a client’s formulation, to understand their needs and to work in a trauma-informed manner in order for these placements to be a success. This work sits alongside specialist nursing, psychology and probation provision to reduce re-offending behaviour and supports successful re-integration into the community.
