Rehab & Recovery (NHS Hub)
The Recovery Service is a multidisciplinary ‘transitional’ team of care co-ordinators, recovery workers, peer recovery workers, and allied professionals (speech and language therapist, dietician, harm reduction worker, and psychologists). It is a rare example of voluntary and statutory (NHS) sector workers integrated as one team and has successfully provided the co-ordination and move on support for patients from the two NHS recovery wards for more than 6 years.

About the service
Inspire North works in partnership with LYPFT to provide care coordination to clients through the Rehab & Recovery (R&R) pathway. This partnership style provides a diverse and holistic approach to client care, with a strong culture associated with recovery principles and ways of working.
Our care coordinators are based within the Recovery Centre as part of a multi-disciplinary team. They support clients with complex mental health needs, who are currently an inpatient at either Asket Croft or Asket House, whilst they work towards developing their independent living skills. Our care coordinators continue to provide support during a client’s transition back into the community, and for up to 6 months following discharge.
