Supported living service for those ready to be discharged from acute care in Leeds. It comprises of four self-contained, fully furnished flats and is located in South Leeds. It is intended as a short-to-medium-stay accommodation service which has throughput to help relieve pressure on Leeds-based acute wards.

East Grange is a supported living service for those with mental health needs in Leeds. Our building consists of 4 fully-furnished flats, a communal garden and a small office for our support staff. We are open 9am-9pm, 7 days a week and our team consists of 6 mental health support workers, 1 team leader and 1 service manager.

Aims of the service:

Our central aim is to continue to support client recovery, post hospital discharge. We aim to support our clients to move into more permanent independent accommodation in the community.

The support we offer:

Initially, we will focus on supporting individuals with their transition from an acute setting back into the community. Living at East Grange is an opportunity for clients to work towards living more independently. We have broken down the types of support we offer into three categories:  practical support, emotional support and social support.
