About the service

The Calderdale Dementia Hub is a partnership between Community Links and Age UK Calderdale & Kirklees and offers an information and advice service for anyone of any age diagnosed with dementia, and their friends, family and carers.

The Calderdale Dementia Hub is commissioned by Calderdale Council and launched in March 2022. The launch of the Calderdale Dementia Hub follows the success of its sister service The Kirklees Dementia Hub, which has provided dementia information and advice to the Kirklees area since 2017.

Aim of the service

  • Provide dementia information and advice to those who are concerned about themselves or loved ones. There is a phone advice line available on 01422 399 833.
  • Provide one to one appointments with you and your loved ones, whether that is by a home visit, phone appointment or in a community setting.
  • Provide community sessions to help people learn more about dementia and how to adapt, live well and remain independent.
